What the Fathers say
Eastern Fathers
John Chrysostom
But that you may learn that there is no absolute necessity for offences, hear what follows, If thy hand or thy foot offend thee, & c. This is not said of the limbs of the body, but of friends whom we esteem as limbs necessary to us; for nothing is so hurtful as evil communications.
Theophylact of Ohrid
Wherefore if thy hand or thy foot cause thee temptation, cut it off, and cast it from thee: it is better for thee to enter into life halt or maimed, rather than having two hands or two feet to be cast into everlasting fire. And if thine eye cause thee temptation, pluck it out, and cast it from thee: it is better for thee to enter into life with one eye, rather than having two eyes to be cast into the gehenna of fire. Desert Fathers
A brother asked Poemen, 'What is the meaning of the text, "Whoever is angry with his brother without a cause" (Matt. 5:22)?' But if anyone wants to separate you from God, then you must be angry with him.'
Western Saints
Jerome

It is better for you to enter life with one eye than to have two eyes and be thrown into the fire of hell. Indeed, it is necessary for scandals to come, but woe to the person through whom the scandal comes! …
Thomas Aquinas
You say that woe to that man by whom the scandal comes; hence scandal should not be inflicted on little ones. And although it should not be inflicted, they nevertheless should not be negligent in avoiding scandal; indeed one can avoid it through something useful for action, or for knowledge, or for support. Hence he sets forth under the likeness of members of the body: and if your hand or your foot scandalize you, cut it off, and cast it from you. …
